We ask for the contact details of a health professional with every referral. This can be an email address or phone number. It helps us make sure you're getting the right support.
This could be your midwife, health visitor, family support worker or another professional who supports you and your family.
If you're a health professional making a referral, we'll list you as the named professional contact while support is in place. This means you'll need to stay actively involved with the family. If you won't be actively involved, please provide a professional who will be.
Please note, this service is currently only available to families living in England and Wales.
